Step 2 - Schedule a Doctor Appointment

If you qualify for a card you will need to find a doctor and schedule an appointment. The normal cost is typically  ~$150.

Qualifying Conditions

​

  • Chronic Pain

  • Cancer

  • Glaucoma

  • Immunodeficiency Virus 

  • Cachexia or Wasting Syndrome

  • Intractable Nausea or Vomiting

  • Epilepsy 

  • Multiple Sclerosis

  • Crohn's Disease

  • Painful Peripheral Neuropathy

  • PTSD

Step 3 - File Your Paperwork & Get Your Card

​

Once you have a referral from the doctor we will make sure your application is submitted correctly with the state.
